This will allow you to address any new or existing injuries or pains without having to go into a medical office. Do not allow the current situation to stop your progress! Prior to your first visit, please print out and complete the New Massage Client or New Physical Therapy Patient paperwork provided below. If you would prefer to complete the paperwork in the office, please arrive 15 minutes prior to your scheduled appointment time.

What To Bring:

Please bring your completed new patient/client paperwork, and if you gave any relevant medical reports or documentation related to your condition (MRI, x-rays, physician notes, physician referral/prescription, etc.)

Location/Parking:

Our office is located at 112 South Old Statesville Road Suite 110 in Huntersville between Crafty Beer Guys and the Huntersville Fire Department. If using navigation, please make sure to type in 112 SOUTH Old Statesville Road to ensure you receive the correct directions. Turn onto Greenway Drive to access parking lot behind the building. You may park in any available space including the spaces marked as “Reserved” and “Visitor.” The entrance is located in the back of the building.
